Using this form ensures we get all the necessary details up front, allowing us to verify records and contact the patient quickly.

We’re currently booking new patients about 4 months out. We’ll keep your office and your patients informed every step of the way.

What to expect after you submit a new patient referral

Once we receive your referral, here’s what happens:

  1. We’ll review and verify the records. If anything is missing, we’ll reach out to your office as soon as possible.

  2. Once complete, we’ll contact the patient within 3 business days to schedule an appointment.

  3. Please note: We’ll attempt to reach the patient a minimum of 3 times before asking for your help

Allowing us to schedule with the first available provider can often get your patient seen sooner. Currently, we’re booking new patients 4-5 months out.
