Using this form ensures we get all the necessary details up front, allowing us to verify records and contact the patient quickly.

Once the form is completed, please print and fax it, along with the patient records, to the contact here:

PLEASE NOTE
As a reminder, please send all information securely. We value patient privacy and want to protect it.

If you prefer to not use our form, please fax the following information to our office:

  • Patient's full name
  • Patient's date of birth
  • A clear, legible copy of the patient's insurance cards (front and back) Reason/diagnosis for referral
  • Any recent office notes
  • Clinical notes regarding referred condition, such as how long symptoms have been present and treatments tried in the past
  • All relevant lab results, imaging, and medical records
  • Referring provider's contact information, including NPI, Address, Contact person and phone

We’re currently booking new patients about 4 months out. We’ll keep your office and your patients informed every step of the way.

What to expect after you submit a new patient referral

Once we receive your referral, here’s what happens:

  1. We’ll review and verify the records. If anything is missing, we’ll reach out to your office as soon as possible.

  2. Once complete, we’ll contact the patient within 3 business days to schedule an appointment.

  3. Please note: We’ll attempt to reach the patient a minimum of 3 times before asking for your help

Allowing us to schedule with the first available provider can often get your patient seen sooner. Currently, we’re booking new patients 4-5 months out.
