Please verify with your specific plan administrator that we are listed in network prior to coming to your appointment.
We cannot see patients with Medicaid Primary or Medicaid HMO/MCP, do not see non-established patients with Medicaid secondary, and do not participate with Workers Compensation.
You are responsible for paying your co-pay or co-insurance at time of service.
Please bring your insurance card(s) and a picture ID with you to each visit.
Responsibility for payment is your obligation and unless prior arrangements have been made, payment is expected at time of service.
If you are not prepared to pay at time of service, your visit may be rescheduled.
