Steps for Making An Appointment with Piedmont Arthritis Clinic
STEP 1
Download the New Patient Form and complete all sections.
STEP 2
Have the New Patent Form signed by your referring physician.
STEP 3
Call the Piedmont Arthritis Clinic Appointment Desk at (864) 235-8396 to schedule an appointment.
Calls to our Appointment Desk typically are returned within two (2) business days. Please allow three weeks for the first available appointment.
STEP 4
Bring the New Patient Form, your insurance card(s) and a photo ID with you to your appointment.
