For New Patients:

Office Hours

Our office is open from 7:30 a.m. to 5:00 p.m., Monday through Friday.

Phone Hours

You may reach our office by telephone from 8 a.m. until 4:30 p.m.

We are closed from 12:00 p.m. until 1 p.m. for lunch.

Any call received after 3:30 p.m. may be returned the next business day.

We make every attempt to answer calls as they come in, however we have a very high call volume. If you are transferred to voicemail, please leave all pertinent information and we will take care of your issue as quickly as possible.

Prescription Refill Request

If you need a refill on your prescription, please call during regular telephone hours or use our website to submit your request. Our office has a 48-hour turn around on prescription refill requests.

No Shows & Cancellations

We request 24 hours notice when canceling or rescheduling your appointment. Failure to do so could result in a “no show” charge and/or discharge from our practice.

Billing and Insurance

Our physicians participate with most insurance plans (see below). In the event that we do not participate with your insurance plan, you will be expected to pay for your visit in full.

As a courtesy to you, we are happy to file your claim so that you might be reimbursed.

Co-pays, deductible amounts, and co-insurance are expected to be paid at the time of service. If there is a balance after your insurance payment is received, payment is due immediately upon receipt of your bill.

If you do not have insurance or have a balance due, you will be expected to pay for your office visit before you see the physician.

New Patient Forms

Confidentiality of Your Medical Records and Personal Information. Information that we receive on any patient is strictly confidential.

For the forms you will need to complete prior to your appointment.


Patients are financially responsible for all charges incurred.  If, for any reason,  your insurance company denies payment or reclaims a payment previously made, the patient will be held financially responsible.  All charges must be paid in full upon receipt of a billing statement, unless financial arrangements have been made in advance.
